Intent to Play/Reimbursment Option

We communicated last week that Eagledale Little League has postponed the 2020 Season until July 1st.

Normally our regular Season ends in June for most of our players so we understand that playing in July may not be possible for every family. We need ALL Registered members to complete the following form. This form must be completed by June 1st, if it has not been completed it will default to a credit on your account. If you decide after June 1st that you wish to cancel we will still issue you a credit minus the uniform cost of $35

Please Note: If you have more than one child and you still owe money for one but paid for the other one please complete multiple forms.
